Lesões endo-periodontais, uma revisão: diagnóstico, prognóstico e tratamento

Abstract

The relationship between periodontal and pulp tissues has been researched and discussed for many years, especially with regard to their influences on health and disease. The aim of this study was to perform a literature review on diagnosis, prognosis and treatment of endo-periodontal lesions. Endodontic-periodontal lesions are caused for inflammatory products present in different rations in these tissues, being a polymicrobial disease, with predominance of anaerobic bacteria, and with a diverse pathogenesis. Represent a challenge for the dentist, as they are common and difficult to clinical diagnose due to the embryonic, anatomical and functional interconnections that exist between the dental pulp and the periodontium, in addition to the fact that two pathologies are present in the same tooth, with independent developments or not. The differential diagnosis between endodontic and / or periodontal origin is extremely important for an adequate treatment, considering the existence of various signs and symptoms that allow the distinction. The correct diagnosis can be made using several resources and mainly through the history of the disease, clinical and radiographic examination. Many classifications have been described to assist in this diagnosis. Therapy consists of endodontic and / or periodontal treatment, performing a planning with a coherent sequence, whether or not there are time intervals between them. Advances in new techniques and differentiated materials, such as guided tissue regeneration and other alternative forms, have increased the treatment options available, providing a superior prognosis, which is variable and dependent on factors such as the quality of therapy and the individual's response.
