Title: Análise radiográfica da posição da cabeça da mandíbula e angulação da eminência articular em indivíduos com má oclusão de classe I e II divisão 1

Resumo: O objetivo deste estudo foi analisar a posição da cabeça da mandíbula e a angulação da eminência articular em indivíduos portadores de má oclusão de classe l e ll divisão 1. Radiografias panorâmicas modificadas para ATM de 264 pacientes pré-ortodônticos com idade entre 7 e 60 anos foram digitalizadas em um scanner Hewlett Packard F4180. A posição da cabeça da mandíbula foi determinada de acordo com a classificação de Gelb e a angulação da eminência foi mensurada de acordo com o plano horizontal de Frankfurt e uma linha de referência que tangenciou o teto da fossa articular e a margem mais inferior da eminência articular nas imagens sagitais de boca fechada. Os testes qui-quadrado, McNemar-Bowker, t de Student e de correlação de Pearson foram utilizados para analisar os dados. Foi observada diferença na posição da cabeça da mandíbula no lado direito (p=0,007) entre ambos os grupos e as amostras com má oclusão de classe II mostraram uma tendência (p=0,039) para uma posição assimétrica entre os lados. A inclinação da eminência articular nos pacientes com má oclusão de classe I mostrou diferença entre os lados (p=0,010), o que não aconteceu no outro grupo (p=0,391). A diferença entre os grupos ocorreu na angulação do lado esquerdo nas crianças com idade entre 7 e 12 anos (p=0,036) e os lados direito (p=0,022) e esquerdo (p=0,002) nos adultos de 20,1 a 60 anos. Não houve correlação entre overjet e inclinação da eminência para os lados direito (p=0,076) e esquerdo (p=0,154). Portanto, foi possível concluir que a localização normal da cabeça da mandíbula foi mais prevalente em ambos os grupos no lado direito e os pacientes com má oclusão de classe II divisão 1 apresentaram uma eminência articular mais angulada no lado esquerdo nas crianças com idade entre 7 e 12 anos e em ambos os lados nos adultos de 20,1 a 60 anos.
Abstract: The objective of this study was to analyze the position of the head of the mandible and the articular eminence angle in Class I and Class II division 1 malocclusion samples. Modified for TMJ panoramic radiographs of 264 pre-orthodontic patients aged between 7 and 60 years were digitized on a Hewlett Packard F4180 Scanner. The position of the head of the mandible was determined according to Gelb’s template and the articular eminence angle was measured according to the Frankfurt horizontal plan and a line connected from the deepest point of the roof of the fossa and the top of articular eminence in mouth closed sagittal images. Chi-square, McNemar-Bowker, Paired Student t tests were applied, and Pearson product moment correlations were determined after measurements on both sides were obtained. It was observed difference in head position of the mandible on the right side (p = 0.007) between both groups. Class II division 1 malocclusion samples showed a trend (p = 0.039) to a more asymmetrical position between sides. The inclination of the articular eminence in Class I malocclusion patients showed differences between sides (p = 0.010), which was not observed in the other group (p = 0.391). The difference between groups was verified in angulation of the left side in children aged 7 to 12 years (p = 0.036) and right side (p = 0.022) and left side (p = 0.002) in adults from 20.1 to 60 years. There was no correlation between overjet and articular eminence inclination. Therefore, it was concluded that the normal position of the head of the mandible was more prevalent in both groups on the right side and Class II division 1 malocclusion samples showed steeper eminence on the left side in children aged 7 to 12 years and on both sides in adults from 20.1 to 60 years.
