Title: Barreiras à adesão dos profissionais de saúde às recomendações do bundle de prevenção de Infecção do trato urinário: uma revisão integrativa

Resumo: Objetivo: Investigar na literatura as barreiras para a adesão dos profissionais de saúde ao bundle de prevenção de infecção do trato urinário na assistência hospitalar. Método:Trata-se de uma revisão integrativa que utilizou o mnemônico PICo (população, interesse e contexto) para elaboração da pergunta de pesquisa e as diretrizes Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) para descrever o processo de seleção dos estudos e garantir transparência no relato. As buscas foram realizadas nas bases Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) e PubMed via MedLine. Desse modo, foram incluídos artigos publicados nos últimos cinco anos (do ano de 2021 a 2025), nos idiomas português ,inglês e espanhol, com texto completo disponível e que apresentasse relação com o objetivo do estudo. Não foi aplicado critério de exclusão. Resultado: Foram identificados 674 estudos nas bases de dados pesquisadas.Destes,154 foram excluídos por se encontrarem em duplicidade ou por não estarem disponíveis na íntegra.Em Seguida,462 foram excluídos de acordo considerando os critérios de inclusão. Após a leitura dos títulos e resumos, 54 artigos foram excluídos por não responderem à questão norteadora. Ao final, 4 artigos foram considerados potencialmente elegíveis para compor a análise final. As principais barreiras identificadas no processo de implementação dos bundles foram: ausência prévia de instrumento padronizado, resistência à adesão por parte dos profissionais, alta demanda de procedimentos, falta de treinamento contínuo e necessidade de capacitação prévia, conhecimento insuficiente ou deficiente sobre medidas de segurança do paciente.Os achados foram organizados em quatro categorias: impacto dos protocolos e checklists na redução de infecção do trato urinário, adesão da equipe aos protocolos, educação permanente e treinamentos e fatores relacionados à segurança do paciente. Conclusão: a adesão aos bundles de prevenção da infecção do trato urinário é influenciada principalmente por barreiras relacionadas à fatores organizacionais e ao conhecimento e engajamento dos profissionais. Estratégias como investimento na capacitação e treinamento, apoio institucional e fortalecimento da cultura de segurança favorecem a adesão aos protocolos e qualificam a assistência.
Abstract: Objective: investigate in the literature the barriers to the adherence of health professionals to the urinary tract infection prevention bundle in hospital care. Method: This is an integrative review that used the mnemonic PICo (population, interest and context) to prepare the research question and the guidelines Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) to describe the selection process of the studies and ensure transparency in the report. The searches were carried out in the Latin American and Caribbean Literature in Health Sciences (LILACS) and PubMed databases via MedLine. Thus, articles published in the last five years (from 2021 to 2025) were included, in Portuguese, English and Spanish, with full text available and related to the objective of the study. No exclusion criteria were applied. Result: 674 studies were identified in the databases surveyed.Of these, 154 were excluded because they were in duplicity or because they were not available in full. Next,462 were excluded according to considering the inclusion criteria. After reading the titles and abstracts, 54 articles were excluded for not answering the guiding question. At the end, 4 articles were considered potentially eligible to compose the final analysis. The main barriers identified in the bundle implementation process were: prior absence of standardized instruments, resistance to adherence by professionals, high demand for procedures, lack of continuous training and need for prior training, insufficient or deficient knowledge about patient safety measures. The findings were organized into four categories: impact of protocols and checklists on the reduction of urinary tract infection, adherence of the team to protocols, permanent education and training and factors related to patient safety. Conclusion:Adherence to urinary tract infection prevention bundles is mainly influenced by barriers related to organizational factors and the knowledge and engagement of professionals. Strategies such as investment in training and training, institutional support and strengthening the safety culture favor adherence to protocols and qualify assistance.
