Title: Avaliação do preparo de canais radiculares ovais por instrumentos de níquel-titânio: estudo por microtomografia computadorizada

Resumo: O sucesso do tratamento endodôntico depende da limpeza e da modelagem do sistema de canais radiculares (SCR). Objetivo: avaliar a eficiência de dois sistemas de limas de níquel-titânio no preparo de canais radiculares ovais, quanto à variação de volume, área de superfície e paredes não preparadas, por meio de microtomografia computadorizada (micro-CT), ao longo do canal e a 5mm do ápice radicular, assim como o transporte e centralização do canal. Material e Métodos: Quarenta caninos ovais com canal único foram selecionados e pareados em dois grupos (n=20) de acordo com o sistema de instrumentação utilizado: XP-endo Shaper e Mtwo. Os dentes foram escaneados por micro-CT antes e após a instrumentação, que foi realizada por uma única endodontista e utilizando-se uma cuba térmica a 37°C. A avaliação dos parâmetros analisados foi realizada por processamento e sobreposição de imagens. Resultados: no terço apical, verificou-se diferença significativa entre os grupos (p<0,05 – Mann-Whitney). Enquanto XP-endo Shaper apresentou maior volume e área de superfície, Mtwo demonstrou maior percentual de paredes não preparadas. Considerando o canal em toda a sua extensão, não houve diferença entre os grupos (p>0,05). Quando comparadas as avaliações entre o terço apical e o canal em toda a extensão, verificou-se diferença (p<0,05 – Wilcoxon) na variação da área de superfície, volume e paredes não preparadas para XP-endo Shaper e Mtwo, com maiores valores para o terço apical. Conclusão: o sistema XP-endo Shaper mostrou-se mais eficaz na instrumentação de canais ovais quando comparado ao sistema Mtwo, produzindo maior aumento de volume, área de superfície e com menor porcentagem de paredes não preparadas nos canais. Em se tratando do e transporte apical e centralização do preparo, não houve diferença estatística entre os instrumentos avaliados (p>0,05).
Abstract: Successful endodontic treatment depends on cleansing and shaping the root canal system (SCR). Objective: To evaluate the efficiency of two nickel-titanium file systems in the preparation of oval root canals for volume variation, surface area and unprepared walls by means of computed microtomography (micro-CT) along the canal. and 5mm from the root apex, as well as the transport and centralization of the canal. Material and Methods: Forty single-canal oval canines were selected and paired in two groups (n = 20) according to the instrumentation system used: XP-endo Shaper and Mtwo. The teeth were scanned by micro-CT before and after instrumentation, which was performed by a single endodontist and using a thermal vat at 37 ° C. The evaluation of the analyzed parameters was performed by image processing and overlay. Results: in the apical third, there was a significant difference between the groups (p <0.05 - Mann-Whitney). While XP-endo Shaper had higher volume and surface area, Mtwo showed higher percentage of unprepared walls. Considering the canal in all its extension, there was no difference between the groups (p> 0.05). When comparing the apical third and canal evaluations over the fullest extent, a difference (p <0.05 - Wilcoxon) was found in the variation of surface area, volume and walls not prepared for XP-endo Shaper and Mtwo, with higher values for the apical third. Conclusion: The XP-endo Shaper system was more effective in the instrumentation of oval canals when compared to the Mtwo system, producing greater volume increase, surface area and a smaller percentage of unprepared canals in the canals. Regarding the apical transport and preparation centralization, there was no statistical difference between the evaluated instruments (p> 0.05).
