Resumo: A doença renal crônica (DRC) é um problema global de saúde pública. No Brasil, cerca de 126 mil pessoas realizam terapia renal substitutiva, mais do que o dobro do que havia no início do século passa do. Esta população é predominantemente idosa, apresenta diversas comorbidades e utiliza polifarmácia. Esses fatores, aliados ao comprometimento renal, expõem essa população aos problemas relacionados a medicamentos (PRMs), que podem interferir no resultado terapêutico. F erramentas direcionadas ao manejo de PRMs em renais crônicos são importantes, porém, tem sido pouco exploradas no Brasil. Nesse contexto, o presente estudo teve como objetivo r ealizar a adaptação transcultural e validação do instrumento PAIR Pharmacotherapy Assessment in Chronic Renal Disease) para o Brasil. O PAIR consiste numa lista de PRMs considerados clinicamente significativos na DRC, originalmente desenvolvido no Canadá, que funciona como um check list para que o farmacêutico possa av aliar a farmacoterapia desses pacientes. O estudo foi realizado em duas etapas. Na primeira, o PAIR foi submetido aos procedimentos de adaptação transcultural seguindo padrões internacionalmente aceitos: tradução, síntese, retrotradução, análise por um com itê de especialistas e pré teste da versão preliminar. Após realização de ajustes de palavras e exclusão de 6 PRMs, devido à não aplicabilidade ao contexto brasileiro, foi obtida a versão final do PAIR em português. Na segunda, o instrumento traduzido e ad aptado foi avaliado quanto à confiabilidade, a validade conceitual e a responsividade, sendo realizado estudo observacional longitudinal retrospectivo. Também foi avaliada a prevalência de PRMs em uma amostra de indivíduos com DRC em tratamento conservador . Uma média de 1 ,26 ± 0,96 PRMs/ paciente foi identificada com o PAIR, sendo a categoria mais prevalente “interação e medicamento tomado inadequadamente” (34,1%) 34,1%). Abstract: Chronic kidney disease (CKD) is a global public health problem. In Brazil, about 126,000 people undergo renal replacement therapy, more than twice as much as there was at the beginning of the last century. This population is often elderly, has multiple comorbidities and takes polypharmacy. These factors, with renal impairment, expose this population to drug related problems (DRPs), which may interfere with the thera peutic outcome. Tools directed to the management of DRPs in chronic renal patients are important, however, they have not been explored in Brazil. In this context, the present study aimed to perform the cross cultural adaptation and validation of the PAIR ( Pharmacotherapy Assessment in Chronic Renal Disease) instrument for Brazil. The PAIR comprises a set of DRPs considered clinically significant for CKD, originally developed in Canada, which works as a check list so the pharmacist can assess pharmacotherapy of these patients. The study was carried out in two stages. In the first, the PAIR was submitted to the procedures of cross cultural adaptation following internationally accepted standards: translation, synthesis, back translation, analysis by a committee of experts and pre test of the preliminary version. After adjusting words and excluding 6 DRPs, due to the non applicability to the Brazilian context, the final version of the PAIR in Portuguese was obtained. In the second, the translated and adapted inst rument was evaluated for reliability, conceptual validity and responsiveness being performed a retrospective longitudinal observational study.
